You don't have to pay for a healthcare directive. The Minnesota Secretary of State's website has a form available for free: https://www.ag.state.mn.us/Consumer/Handbooks/Probate/HealtCareDir.pdf

The form is just that--it's a form and it not specific to any individual. Obviously, it does not come with any legal advice which you may want or need.
